EXCHANGE CARD SIGNATURE

It's great that you are preparing for the birth of a child at the Kyiv Perinatal Center!
After 24 weeks, we are waiting for you to sign the exchange card!
You will meet an obstetrician-gynecologist who coordinates and provides coordinated and high-quality medical care for the expectant mother. His tasks go beyond the usual examination and are aimed at organizing the entire process of preparation for childbirth.
1. Initial consultation and assessment:
- Getting to know the patient: The doctor establishes contact with the expectant mother, collects anamnesis (pregnancy history, previous diseases, features of the course of this pregnancy).
- Risk assessment: Based on the anamnesis and examination results, the doctor assesses possible risks and determines the patient’s risk group.
- Determining the patient’s needs: The doctor finds out the woman’s individual needs, wishes and expectations regarding pregnancy and childbirth.
2. Development of an individual plan for pregnancy and childbirth:
- Drawing up a plan for examinations: The doctor determines the necessary volume of diagnostic procedures (tests, ultrasound, CTG, consultations of narrow specialists) and monitors their timely implementation.
- Consultation planning: The doctor coordinates visits to other specialists (therapist, endocrinologist, cardiologist, etc.) according to indications.
- Development of a preliminary birth plan: Based on the woman’s health status and her wishes, the doctor discusses possible birth scenarios.
3. Coordination between different services and specialists:
- Interaction with other doctors: The doctor maintains contact with the doctor who directly manages the pregnancy, the obstetrician-gynecologist of the maternity ward, the anesthesiologist, the neonatologist and other specialists.
- Organization of consultations: In complex cases, organizes consultations with the participation of various specialists to make the optimal decision.
- Ensuring the transfer of information: The doctor is responsible for the timely transfer of all necessary medical documentation between different departments of the medical institution.
4. Informational and psychological support:
- Providing information: provides the patient with complete and reliable information about the course of pregnancy, preparation for childbirth, possible complications and methods of their prevention.
- Consulting on various issues: answers the woman’s questions, helps dispel fears and anxieties associated with pregnancy and childbirth.
- Referral to childbirth preparation courses: If necessary, may recommend attending specialized courses for future parents.
5. Organizational issues:
- Appointment and examination booking: can help with arranging appointments for necessary visits and diagnostic procedures.
- Medical documentation: monitors the correct execution of all necessary medical documentation.
- Hospitalization preparation: In the case of planned hospitalization, the doctor coordinates this process.
The main goal is to provide the patient with an individual approach, coordinate the work of all involved specialists and services, and provide the necessary information and psychological support so that the expectant mother feels confident and calm on the eve of childbirth.
It is a kind of “bridge” between the patient and the medical system, helping her navigate all stages of preparation for childbirth.
