– State registration of the birth of a child;
– Registration of the place of residence of the born child (during martial law, the service is not provided);
– Assignment of assistance at the birth of a child;
– Registration of the born child in the State Register of Individuals – Taxpayers;
– Determination of the origin of a born child whose parents are not married to each other;
– Determination of the child’s belonging to the citizenship of Ukraine;
– Assignment of a unique record number to the child in the Unified State Demographic Register (UNZR).

The eMalyatko application can be submitted by:
– Married parents;
– Unmarried parents;
– Single mothers;
– The application can only be submitted during the stay at the Kyiv Perinatal Center.

1. Application (formed by the administrator using the software of the “Diya” portal).
2. Original passports of parents or digital passports in the “Diya” application.
3. Number of the generated electronic medical certificate of birth or medical certificate of birth (form N 103/o).
4. To fill out the application, you will also need the following information:
– Document on assignment of the registration number of the taxpayer’s registration card of the subject of the application (if available) (it is enough to have a scan copy or a photo on your phone);
– Marriage certificate (it is enough to have a scan copy or a photo on your phone);
– UNZR of one of the parents – to order the service of assigning the UNZR of the child (this number is indicated in the passport of the citizen in the form of an ID card or in the passport of a citizen of Ukraine for traveling abroad with a contactless electronic carrier (it is enough to have a scan copy or a photo in the phone);
– Current account number (IBAN) with a special mode of use “eSupport” (for the appointment of assistance at the birth of a child);
– Current account number (IBAN) opened in JSC CB “PrivatBank” (to receive the service of providing monetary compensation for the cost of one-time in-kind assistance “baby package”) (optional). The card is obtained in person by one of the parents at any branch of JSC CB “PrivatBank”.
6. If the eMalyatko application is submitted by one of the child’s parents (in case the second parent is not present, it is necessary to additionally submit:
– Notarized consent of the other parent to assign the child a surname – if the parents have different surnames;
– Application of the mother and father of the child about recognition of paternity – if the child’s parents are not married.
7. Method of obtaining the service: In person.

– State registration contradicts the requirements of the legislation of Ukraine;
– A request for state registration was made by an incapacitated person or a person who does not have the necessary powers for this;
– In the event that the applicant provides false or unreliable information.

What is a UNZR?
– A unique registration number in the registry (UNZR) is a digital, unchangeable, lifelong identifier of a person.
– This number will belong only to you or your child throughout your life and will allow you to quickly and reliably establish your identity.

Why is a UNZR needed?
A UNZR is needed to facilitate the interaction of the state, business and citizens. All information about you or your child will be stored in a single state database.

How to get a UNZR?
A UNZR is automatically created:
– When receiving a passport in ID card format;
– When receiving a biometric international passport;
– When receiving the “eMalyatko” service.

Childbirth allowance
Every family in which a child is born has the right to receive financial assistance. The amount of the allowance does not depend on the number of children or the financial situation of the family.
The amount of payments is 41,280 UAH. Payments are made in two stages:
– 10,320 UAH. is paid within one or two months from the date of application.
– 30,960 UAH. is paid in equal installments of 860 UAH per month for three years.
